For hospital discharge, accommodation transitions, unexpected breakdown of existing supports and other time-sensitive circumstances, Waratah Connects can explore priority intake and rapid commencement of supports.

We work with participants, families, Support Coordinators, case managers, hospitals and other professionals to understand what is needed and determine how support can be established safely and as quickly as possible.
